Any student looking to play baseball must

complete the following prior February 17th:

1.  Pass a Physical

     You must have a passed physical on file with the school nurse

     to be cleared to play.  You can either get this done with your

     personal doctor or with the school based nurse.

2.  Parent Signature for Participation

     You must have a parent or legal guardian sign a permission

     form for you to play high school sports.

3.  $60 Athletic Fee

     This is a mandatory fee in which you pay to the school

     treasurer.  Checks must be payable to Montbello High

     School.

If you try out for the team, and do not make it, you will get a refund for the

athletic fee that you paid.
